10 Ways to Make Your Home More Accessible Without Sacrificing Style

 

Most homes are not designed with the needs of older adults in mind. The use of universal design features make everyday activities easier for everyone, regardless of their age or ability. That’s because good design is functional design for all. And the great news is, it looks beautiful too!

When you schedule an in-home consultation, you can expect that we will:

  • Apply our expertise in occupational therapy and aging to conduct an assessment of your individual needs and goals

  • Evaluate interior and exterior elements of your home, using a checklist based on age-friendly and universal design principles

  • Provide recommendations on how to make your home accessible, safe, and functional so that you can remain active and independent in a familiar and cherished place

  • Offer to work with your architect, designer, or builder to represent your requirements and facilitate the inclusion of universal design features

We also consult directly with architects, designers, builders, and real estate agents and developers to provide age-friendly certification of plans and/or properties.​

PATIENT NAVIGATION OF HEALTHCARE & SOCIAL SYSTEMS

When you experience a change in your health, it can be difficult to feel in control, or that your voice is being heard. With in-depth knowledge of the healthcare system and community resources, we can help you make informed decisions about your health, and access the services you need.

Whether you reach out to us from home or from the hospital, we will:

  • Establish a relationship with you, your caregivers and healthcare providers

  • Develop an individualized, person-centred care plan based on an assessment of your health and/or functional goals

  • Discuss public versus privately funded programs and services

  • Help you choose and coordinate the equipment, care and service options that are right for you

  • As applicable, we can:

    • Accompany you to (medical) appointments

    • Support communication between you and your care team

    • Promote self-management via action-oriented coaching and education

    • Assist with transitions in care or end-of-life/palliative care

Inclusive Aging complies with the Personal Health Information Protection Act, 2004 (PHIPA) and Personal Information Protection and Electronic Documents Act (PIPEDA), which set out rules for the collection, use and disclosure of personal information.

In 2019, I received my 'Advanced Patient Navigation Certificate' from York University's Health Leadership & Learning Network program - and I'm now teaching the course!
